Restoring Function and Aesthetics: Full Mouth Rehabilitation in a Patient with Deep Bite and Generalized Tooth Wear

Patient Story

A 60-year-old gentleman, came to our clinic worried about his teeth. He had been experiencing sensitivity, difficulty chewing, and felt his smile looked “old and worn out.” Over the years, his teeth had become flat and short, making him feel self-conscious. He wanted to restore his smile and chew comfortably again.

Patient’s Words: “I feel like I got my youth back after this treatment!”

1. Patient Details

Age & Gender: 60-year-old ; Male

Main Concerns: Tooth sensitivity, difficulty chewing, worn teeth, and dissatisfaction with smile appearance.

History: No major health issues. Teeth showed heavy wear (attrition) and a deep bite (upper front teeth covering the lower teeth more than normal).

 

2. Findings During Examination

Face & Smile: The lower part of the face appeared shorter due to worn teeth. Wrinkles around the lips and strain while closing the mouth were noticeable.

Inside the Mouth: All teeth had flat, worn surfaces with exposed dentin (the inner tooth layer), a deep overbite, and loss of the natural bite pattern.

3. Treatment Approach

Duration of treatment : 3 months

Step 1: Increasing Bite Height with Temporaries

Relaxed the jaw muscles and recorded the ideal jaw position.

Created a digital model and silicone mould of the teeth.

Temporary crowns were placed using silicone mould by injection moulding technique to gently raise the bite and improve chewing.

This “trial phase” lasted 8 weeks to ensure comfort and proper adaptation.

Step 2: Permanent Restorations

Back Teeth: Strong ceramic on lays (Emax) were placed to rebuild chewing surfaces.

Front Teeth: Full ceramic crowns were placed to enhance the smile and restore proper bite movement.

Step 3: Final Adjustments

Fine-tuned the bite for even contact and smooth function.

Provided a night guard to protect the new teeth from grinding.

 

Step 4: Final Results

Smile & Appearance: The smile looked natural and youthful, the tooth length was restored, and lip support improved.

Chewing & Comfort: The patient reported no sensitivity and could chew comfortably.

By gradually increasing the bite and using advanced ceramic restorations, Patient's smile, chewing ability, and confidence were fully restored. Follow-ups show the results are stable and long-lasting.
