Patient's Age: 66 years

Concern: Complete edentulism in both maxillary and mandibular arches; dissatisfaction with removable dentures

Case duration: 6 months

What Brought the Patient to Us?

The patient sought treatment due to missing teeth in both the upper (maxillary) and lower (mandibular) arches, accompanied by difficulty in chewing food and unsatisfactory aesthetics due to missing teeth. He had been using conventional removable dentures for the last five years but was dissatisfied with their performance and comfort. Motivated to find a more stable and functional solution, he opted for implant-supported fixed prostheses.

What Did We See?

During his dental check-up, we found:

     Complete edentulism

     Removable Denture causing stomatitis on the palate and corner of the mouth

Diagnostic Evaluation

        Imaging: Cone-Beam Computed Tomography (CBCT) scans of both arches

        Implant Planning: Identified potential implant sites across both arches, considering anatomical limitations 

 

STEP-BY-STEP REHABILITATION PROCEDURE

        1. Surgical Phase

Anaesthesia: Local anaesthesia with lignocaine

Implant Placement:

Maxilla: 7 implants strategically placed

Mandible: 8 implants strategically placed

Primary Stability: Achieved torque of 35 N/cm for all implants

Postoperative Care: Standard protocols followed to ensure healing and osseointegration

       2. Prosthetic Rehabilitation

Healing Period: Allowed for adequate osseointegration before prosthetic phase

Impression Taking: Utilized appropriate materials to capture precise implant positions

Prosthesis Design: We fabricated fixed prosthesis tailored to patient's occlusion and aesthetics. We choose Paulo Malo bridge for Mr. Sam on which zirconia crowns were fixed giving him a natural looking smile, thus fixing his lifestyle.

Outcome: Successful restoration of function and aesthetics with high patient satisfaction

He said he finally could chew food in a satisfactory manner and confidently smile again in social gatherings. 

We saw him for follow-up visits, and everything was perfect – no discomfort, and his implants looked well integrated with bone and no signs of infection surrounding the implants. We also explained him proper water flossing protocol to prevent food lodgement beneath the prosthesis to prolong the life of the dental implants.

Final Thoughts
